What to Bring for Your First Visit

To make your first visit go as smoothly and quickly as possible, please bring the following items.

• Insurance cards – medical and pharmacy
• Driver’s license (or another government issued photo ID)
• List of current medications and supplements including dosages
• Enough of your regularly needed medical or nutritional supplies to accommodate a two-three hour visit, as well as your driving time
• Healthcare Proxy and Living Will
• Paperwork mailed to you in advance (or download and print it from the links below)
