Principal Care Management / Principal Illness Navigation

Principal Care Management and Principal Illness Navigation services are available for our patients!

If your clinical team thinks you are eligible and would benefit from one of these services, they will discuss that with you. If you would like more information on these services please ask your clinical team.

Both services are reimbursable through your insurance on a monthly basis when you have received at least 30 minutes of care management services outside of regular office visits. A co-pay may occur.

Principal Care Management (PCM) eligibility:

Patients with a single chronic condition that is expected to last at least three months, are eligible to participate. This focused approach helps to improve health outcomes and reduce the risk of complications.

The Principal Care Manager (PCM) will work with you and your caregivers to:

  • Help manage your medical condition 
  • Ensure timely adjustments to your treatment plan are made based on any symptoms or changes that may arise
  • Coordinate care and services to ensure that you receive the comprehensive care you need. 

Principal Illness Navigation (PIN) eligibility:

Patients that are eligible for these services must have a serious medical condition that puts them at risk for complications.

The Principal Illness Navigator (PIN) will work with you and your caregivers to:

  • Help patients with serious conditions understand their diagnosis and navigate the healthcare system
  • Address barriers to your receiving care
