Oncology Nurse Navigation

Oncology Nurse Navigators (ONN) use their extensive knowledge, specialized skills and good clinical judgment to improve patient care. They guide patients through the healthcare system with individualized assistance to patients, families and caregivers to help overcome health care disparities and barriers to accessing care. ONNs also proactively facilitate close and frequent communication between patients and caregivers to arrange timely medical care.

Who are Your Nurse Navigators? 

HOACNY has a thoracic nurse navigator, working with physician champion Aref Agheli, M.D., and a gastrointestinal nurse navigator, working with physician champion Santosh Kumar, M.D.

Both navigators:

  • are registered nurses as well as Oncology Certified Nurses with several years of oncology experience
  • recognize and address disease complications that may occur during and after cancer treatment
  • are certified in Chemotherapy and Biotherapy administration and current in all oncology treatment modalities

What is the Role of a Nurse Navigator?

  • Support you to better understand and manage your diagnosis through personalized care
  • Help address your needs, including financial and economic, transportation, cultural, social emotional, spiritual, and informational
  • Coordinate all your appointments in a timely manner
  • Recommend and refer you to social workers, dieticians, outreach services and support groups
  • Offer and reinforce education for you and your loved ones about your disease, treatments, side effects, and self-supportive care
  • Reach out to you after each medical treatment to thoroughly assess how you are feeling and offer symptom management, which may include an office visit
  • Help bridge any communication gaps between you and your providers
  • Track the results of your appointments and meet with you on a regular basis
